Care at Home Program

The grant regards the creation of a pilot program aimed at the physical, mental and psychosocial strengthening of dementia patients residing in Patra. This Care at Home Program will target seniors recently diagnosed with stage one and two dementia, as well as their caregivers. The purpose of this Program is to prolong the presence of these individuals at home. The Program involves the creation of a three member team: a social worker, a psychologist and a physical education expert. The services offered are: mental strengthening exercises relevant to the mental health assessment; psychological support to both the elder and the caregiver; and physical strengthening exercises along with specialized advice on nutritional regime. The team visiting the homes will simultaneously train the caregivers to be able to implement the services offered themselves. During the six months of the pilot phase, the number of beneficiaries will be at minimum 20.
