The Association’s purpose is to develop conditions and services for the improved quality of life of patients with dementia and their caregivers. The Association cooperates with the Municipality of Chalkida and the Prefecture of Evoia for the organization of events, while at the same time it offers services on a volunteer basis to OPCEs (KAPI) and Care Homes in the greater region of Evoia. In 2010 the “Agia Irini” Day Center was established, with the following services provided: dementia prevention programs; non-pharmaceutical interventions for persons with dementia; mental strengthening exercises; education and psychological support of caregivers of persons with dementia; memory tests and neuro-psychometric tests. The Association also provides daily telephone support, carries out information campaigns in schools regarding Alzheimer’s disease and takes part in research programs. Both the Association and the Day Center are operated by volunteers.
Care at Home Program
The grant regards the creation of a care at home program for patients with dementia. The program will last 13 months and will serve 60 families of dementia patients with home visits. The support for each family will last 4 months, with 20 families being served during each cycle. The program includes a psychologist, a nurse, a social worker, a physical therapist, a neurologist and a case manager-psychologist who will offer various services to persons with dementia and their caregivers. These services include the following: information about Alzheimer’s and other forms of dementia; education on the disease and counselling on patient behaviours; education on individualized patient care; individualized information for settling legal, insurance and other economic matters; psychological support of the relatives of patients with dementia; medical monitoring and other services. In order to achieve a more holistic program, one-off additional services will be provided by volunteers of the program throughout the year. The main goal of the program is to support the family/caregiver of the dementia patient through education so that upon their exit from the program, families will be better able to take care of the patient.
